This page outlines how Marie Veronique may use your approved feedback, photographs, and personal information across its marketing and educational communications, and what your consent authorizes. Please review this information carefully, then return to the enrollment form to indicate whether you consent to the use of your content for marketing purposes.

After completing all four required study surveys (baseline, initial experience, one-week, and overall experience), I will receive a $100 voucher toward a future purchase of $100 or more before taxes & shipping fees at MarieVeronique.com, delivered by email as a reward code within two business days after completing all four surveys. The reward code cannot be combined with other discounts or promotions, redeemed for cash or credit, or applied to previous purchases. My reward is not dependent on providing positive feedback, giving specific survey answers, submitting photos, or posting a public review. I will still receive my earned reward if decide to withdraw that permission later. I understand that I will not receive additional payment or royalties if my approved materials are used for marketing. When required, Marie Veronique will disclose that I received a $100 voucher toward a future purchase and any other relevant benefits, including complimentary products, when sharing my testimonial. If I choose to post publicly about the products myself, I agree to clearly disclose these connections as well.

Changing or Withdrawing Your Permission

I can change or withdraw my marketing permission, consent to share health-related information, or permission for specific types of content at any time by contacting: survey@MarieVeronique.com. If I withdraw permission, Marie Veronique will stop using the affected materials in new marketing communications and remove them from channels it controls as soon as reasonably possible, within 30 days at the latest, or sooner if required by law. Marie Veronique will also notify its agencies and partners to stop using the affected materials and request their removal. The same process applies when the three-year permission period ends. I understand that withdrawing permission does not undo uses that were previously authorized. For example, emails already sent or printed materials already distributed may not be retrievable, and Marie Veronique cannot guarantee removal of copies independently shared by third parties. These limitations do not affect any rights I may have under applicable privacy or consumer protection laws, including rights to request deletion or removal. Records relating to my consent, safety, or legal requirements will only be kept as permitted or required by law, as explained in the privacy notices. For uses I have specifically approved that follow the terms of this form, I agree, to the extent permitted by law, not to bring claims against Marie Veronique or those acting on its behalf for infringement of my publicity, privacy, or intellectual property rights arising solely from those authorized uses. This does not limit my right to bring claims involving product injury, negligence, misleading edits, unauthorized use or disclosure, fraud, intentional misconduct, or violations of the law. I am not giving up any statutory consumer or privacy rights, or rights relating to unknown claims under California Civil Code Section 1542. I am not required to keep my experience confidential, reimburse Marie Veronique for claims, or limit my ability to share honest reviews.

Privacy and Combined Study Findings

Marie Veronique will use my approved materials, identifying details, and record of permission to manage this authorization and share content only as permitted. For more information about how my information is collected, used, shared, and retained, including my privacy rights, I can review: Privacy Notice. I can also contact survey@MarieVeronique.com with questions or privacy-related requests. This form does not authorize the sale of my health information or limit my rights under applicable privacy laws. I understand that this authorization applies only to the individual feedback and personal details I have approved. Separately, Marie Veronique may share combined study findings in accordance with the enrollment privacy notice and applicable law, but only after the information has been de-identified so that it cannot reasonably be linked back to me. Simply removing my name is not enough. My individual quotes or feedback cannot be used as anonymous testimonials unless I have specifically authorized their use.

Final Acknowledgment and Consent

By accepting, I confirm that:

• I am at least 18 years old and have reached the legal age of adulthood where I live.
• The feedback and materials I have provided reflect my genuine experience.
• I have the right to share the materials I submit and will identify any photographer or other person who may hold rights to them.
• I understand that I am giving permission only for the specific materials and uses I have selected.
• I understand how to withdraw my permission.
• I understand that agreeing to marketing use is optional and that declining or withdrawing permission will not affect my completion of the study or any reward I have earned.
• I will receive, or have the option to download, a copy of my completed authorization.
